WHEREAS, in return for instruction in firearms, use of premises and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I have read and I agree to the following(please check all checkboxes for waiver to be submitted):

I agree to release, indemnify, hold harmless and defend St. Lucie Shooting Center, L.L.C, and any of its employees, directors, officers, or agents(hereinafter "SLSC"), from any or all fault, liabilities, costs, expenses, claims, demands, or lawsuits arising out of, related to or connected with: the discharge of firearms: the course of instruction; my participation in the course of instruction; the range, building, and premises used for the course of instruction(hereinafter the "Premises"); my presence on or use of said Premises; and any and all of my acts or omissions. Should any such claim, demand, or lawsuit arise or be asserted in any way whatsoever related thereto, whether arising under the laws of the United States or of and State, or under any theory of law or equity, i will indemnify, hold harmless and defend SLSC from any and all costs, expenses, or liability including, but not limited to, the cost of any settlement or judgement made or rendered against SLSC, whether individually, jointly, or in solido with me, together with all costs of court and other costs or expenses incurred in connection with any such claim, demand, or lawsuit, including attorney's fees.

I waive for myself and my administrators, assignees or heirs, any and all rights and claims for damage, losses, demands and any other actions whatsoever, which I may have or which may arise against SLSC,(including but not limited to any and all injuries, damages, or illnesses suffered by me or my property), which may, in any way whatsoever, arise out of, be related to or be connected with: the course of instruction; the Premises,including any latent defect in the Premises, my presence on or use of said Premises; my property(whether or not entrusted to SLSC); and the discharge of firearms. SLSC shall not be liable for, and I, on behalf of myself and my executor, administrators, assignees, or heirs, hereby expressly release SLSC from any and all such claims.

I expressly assume the risk of entering the Premises and of taking part in the activities on the premises, which include, but are not limited to, instruction in ithe taking part in activities on the Premises, which include but are not limited to, instruction in the use of firearms, the discharge of firearms, and the firing of live ammunition.

I understand that shooting and the use of firearms may be dangerous and may cause serious injury or death. I also understand that injury may occur from many causes, including ricochets or from firearms that I am not using, but are in use on the range while I am also on the range. I knowingly accept such risks and expressly and unequivocally release SLSC from any and all liability in connection therewith

I am familiar with the operation of all firearms that I will use, handle, or shoot, while at SLSC and, I will obtain instruction on the operation of any firearms with which i an not familiar prior to handling such firearms at SLSC.

I am familiar with and understand all range commands that are used at SLSC, and I have had the range commands explained to me, and i will follow and range commands immediately.

I am legally permitted to possess a firearm or ammunition; I am not subject to any injunction that would prevent me from possessing a firearm or ammunition; I have not been convicted of a felony; I am not under any legal disability or legal restriction that would prevent me from possessing a firearm or ammunition; and I am not under the influence of any drugs or alcohol which prevent me from reading and understanding this release.

I hereby acknowledge and agree I have read, understand and will at all time abide by SLSC riles, range rules and procedures.

This instrument binds me and my executors, administrators, assignees or heirs. This is a legal and binding document and you may seek the advice of an attorney or legal counsel prior to signing it.

CORE SAFETY RULES - MUST READ!!!!!!!!

TREAT ALL FIREARMS AS LOADED AT ALL TIMES.

Muzzle Discipline: Always keep your muzzle pointed in the safest direction and NEVER intentionally cross anything you are not prepared to shoot. A safe direction is where NO injury and only minor property damage could occur if an unintentional round is fired.

Trigger Finger: Always keep your finger straight on the frame of the gun unless the muzzle is pointed at something you are prepared to shoot.
The trigger finger is only placed into the trigger guard and on the trigger when coming on target just before firing. When not firing, place your trigger finger straight on the frame. If the trigger fingers is held outside the trigger guard, rather than on the frame, a convulsive hand grip could cause the trigger finger to fire the gun.

Be sure of your target, the surroundings and beyond: While targets on a square range are easily identifiable, threats on the street are often in close proximity or in front of innocent bystanders.

RANGE RULES

It is everyone's responsibility to keep himself, and everyone else safe. Safety must be a personal commitment by everyone at all times.

Any time you observe a potentially unsafe condition developing, it is your responsibility to correct the matter by either contacting the range officer or a staff person of St Lucie Shooting Center or immediately call CEASE FIRE

Upon hearing CEASE FIRE, immediately stop shooting, bench your weapon, pointed down range and back away from shooting point.

Eye & Ear protection are required by those on the firing line and those anywhere on the range.

Wear Proper Clothing: Hot shell casings will be ejected from various weapons. It is recommended(but not mandatory) to wear a closed shirt, no open toe shoes and a hat with a bill. If you are injured, you release and hold harmless St Lucie Shooting center LLC and its employees from damages.

Alcohol, Drugs, and Guns Do NOT mix Persons under the influence of alcohol or drugs will not be allowed to participate in any shooting exercise.

Weapon Malfunction : If there is a problem, malfunction, or you have questions concerning your weapon, place the weapon on the shooting bench, pointed down range in a safe direction, and contact the range officer or member of our staff.

All Equipment: Come to the firing line with ALL your equipment that you will need. Be as practical as you can.

LOAD ALL MAGAZINE AND WEAPONS IN YOUR SHOOTING STALL

DO NOT LOAD WEAPONS BEHIND OTHER SHOOTERS OR ON THE BACK TABLES

Report injuries no matter how minor, to a range officer or staff member of St Lucie Shooting Center as soon as practicable.

Targets: Targets must be mounted onto backers before use. Cardboard and tape is provided on the range.DO NOT USE STAPLES ON TARGETS

Damage: Fines and possible expulsion will be imposed to any shooter that causes damage in the range, including shooting target hangers, partitions, cables, etc. If found that damages were intentional, Criminal Charges will be filed and the shooter will be banned from further use of the facility.

No Tulamo, steel case, steel core, penetrator or green tip ammo allowed on this range.

I HAVE READ, UNDERSTAND, AND WILL FOLLOW ALL OF THE SAFETY AND RANGE RULES SET BEFORE ME
